DESCRIPTION:

SALARY: Minimum of $53,543/yr  (Plus Benefits/IPERS)

The primary purpose of this position is to ensure access to oral health care for underserved populations, with an emphasis on early intervention and preventative oral health care.  The I-Smile Oral Health Program Coordinator is responsible and accountable to manage the I-Smile™ Project.  The Coordinator will serve as the oral health prevention expert and the liaison between public health, families and dental providers to ensure completion of dental care.   Responsibilities include infrastructure building, administrative duties, population-based services, care coordination and gap-filling direct care services.
DUTIES:

Essential Duties:

Develop partnerships and participate in health promotion and planning to strengthen the dental public health system including participation in the needs assessments within Woodbury County.
Link with the local Board of Health for planning, promotion, sharing local data, and assuring local oral health initiatives.
Educate, train and provide guidance to healthcare professionals regarding children’s oral health.
Provide annual training and develop protocols about oral health for all Title V agency staff and additional ongoing training for staff that provide dental care coordination and / or direct dental services.
Establish a referral network of local dentists willing to serve Medicaid clients and / or accept Title V dental vouchers.
Coordinate and complete the county-wide oral health school audit.
Collect data, complete required programmatic documentation and develop reports following program / funder / agency requirements in a timely manner.
Review and update policies and procedures regularly.
Provide oral health education and outreach to community partners, schools, daycares, head start classrooms and at public events.
Attend and participate in work-related meetings, conferences / seminars and committees, including some that are out of town.
Ensure and provide care coordination, assist families in finding a dentist and payment source for dental care, with referral and follow-up to individuals and families.  Evaluate outcomes, effectiveness of plan and make changes as necessary.
Promote and maintain health of individuals, families and the community through provision of oral health gap-filling services including oral health screenings, risk assessments, oral prophylaxis, fluoride varnish applications, Silver Diamine Fluoride applications, oral hygiene instruction/nutrition and tobacco counseling, and dental sealant application especially for at risk individuals.
Provide education, counseling and anticipatory guidance to individuals, families and community groups that are adapted to their age, developmental status, disability, unique needs, lifestyle, cultural and socio-economic situation which promote health and prevent disease.
Perform data entry and understand reporting functions in state reporting data base.
Educate clients and families on their rights as related to privacy of medical information.
Participate in agency, division and program evaluation and assist with development of future goals and objectives.
Assist in agency/division planning and response to disaster situations and emergency situations or events in conjunction with agency emergency plan.
Ensure participant records and communications with participants, co-workers and other health professionals are in accordance with Confidentiality Policy.
Assist with quality improvement process within the division and agency.
Provide customer service that is courteous and responsive.
Assume responsibility for own professional practice, growth and development by participating in continuing educational opportunities, pursuing additional education, attending conferences, and participation in professional organizations, workgroups or agency committees.
Contribute to a work environment that fosters ongoing educational experiences regarding public health practice for colleagues, other health-related students, healthcare professionals and members of the community.
Adhere to applicable Occupational Safety and Health Administration (OSHA) standards, such as those concerning bloodborne pathogens, airborne pathogens, communicable diseases or exposure to other hazards during routine assignments or during public health emergencies.                                                                                           

REQUIRED QUALIFICATIONS:

Education, Experience and Special Requirements:

Current licensure as a Dental Hygienist in the State of Iowa.
Bachelor’s degree required.
Minimum of one year Dental Hygiene practice experience is required.  Experience in maternal/child health, public health, office/clinic setting and/or home health is preferred.
Mandatory Child and Adult Abuse Reporter Certificate or be willing to obtain.
CPR Certification required or be willing to attend training.
Must have valid driver’s license, reliable transportation and ability to obtain car insurance at agency-specified liability level.
